The focus is on creating a caring environment centered on healing and wellness. Nursing practices are based on evidence and current research, as well as hospital policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Responsibilities include coordinating, ensuring, and providing direct patient care activities on the unit. This involves provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also essential. The provision of patient and family-centered education is critical. Care coordination and individualized treatment planning are applicable as needed. Job Responsibilities: Assessment of patient needs is crucial. The RN performs and documents a comprehensive assessment in accordance with their scope of practice and hospital policies. Consideration of the patient’s culture, age, and developmental level is mandatory. Identifying patient risk factors and presenting complaints is essential. Patient health information is obtained from family or others as necessary. Both objective and subjective data are collected. Verification of initial admission assessments is completed if obtained by other healthcare team members. Development of a problem list prioritizes patient needs based on input from the inter-professional team. Actions and interventions are designed relevant to the patient’s culture and age. Desired outcomes are determined in coordination with the patient and family, in accordance with the medical plan of care. Full implementation of patient care therapies and procedures occurs within established policies and guidelines. Procedures, treatments, and processes are explained to the patient and family. Actions are initiated to alleviate the patient’s stress and discomfort. Changes in the patient’s condition are recognized, responded to, and communicated appropriately. Ongoing assessment and response to procedures and therapies are conducted and documented according to hospital policy. Tasks are delegated appropriately, and oversight of team members' performance regarding patient care is provided. The accuracy and completeness of delegated patient care are monitored. Educational needs of patients related to prioritized problems are identified and addressed. The patient discharge process is facilitated by executing discharge orders in a timely manner. Input into patient/nurse assignments is provided while considering available staffing and patient acuity. Active participation in unit-based activities that support organizational values and priorities is demonstrated. Promotion of a positive group dynamic and healing environment through words and actions is essential. Nursing excellence is assured in accordance with accepted standards and evidence-based practice. Engagement in unit quality improvement activities, such as patient satisfaction and safety monitoring, is encouraged. A holistic perspective regarding patient care is maintained.
